About Form 9465
Who fills it in
Use Form 9465 to ask for a monthly installment plan when you can’t pay the full amount shown on your tax return or on an IRS notice.
Where it goes
Attach it to the front of your return. If you have already filed, or you are answering a notice, mail it on its own to the IRS service center listed in the instructions.
Watch out for
- The IRS charges a user fee to set up an installment agreement; it depends on whether you apply online and how you will make the monthly payments.
- You can also apply online with the IRS Online Payment Agreement application.
